You can read the study questions or download them in a Word file. We compiled information about all 1000 companies for 15 of the questions (i.e., the "verification questions"), and we request that you please verify or correct the information that we collected and provide responses to the additional questions by entering the username and password provided in the letter sent to your CEO here:

Please note: We are still following up with companies, and will use the Fortune 2011 list once it is released. We will publish the results of the study in October 2011. This means that we need final information from all companies by August 15, 2011, and approved copy for CEO and company statements by September 1, 2011. We will very much appreciate receiving your information as soon as possible.

We designed the on-line form to make things as easy as possible. The form allows you to enter it as many times as you like and it saves the information for individual pages every time you select "Next" or "Prev" at the bottom of the page. You can return to any page, edit any information, and exit the form any time. Since different people may access the form, albeit at different times, we added a status page so that you can easily keep track of completion of the various parts, and it is very important that you make sure all of the answers on this page say "Yes" when you are finished. You will need to access the form to see the verification information (i.e., the responses to 15 questions that we filled in based on prior responses and our searches of publicly available information).

With respect to the verification questions, we know that many companies offer summer and part-time opportunities for high school students (including relatives of some employees), but that these positions are not generally advertised, so it is very difficult for us to find evidence of job opportunities created for teens. However, these opportunities are very important to teens, and even if you just employ one or two a year, the numbers add up over the entire set of companies. Given this and emphasis on jobs for youth, we ask you to please carefully check the answer to question 2.2. Also, with respect to question 1.1, we are interested in whether you believe that your company has a commitment to children, and it appears in writing in your mission, vision, values, philsophy, culture, responsibility or other statements. If your commitment is to communities or families and you believe that this includes children, then we would count that, but if your company statements only focus on adults or employees and you have no under 18 year old employees, then they would not count.
